This is cyclopia, which iirc is caused by rare genetic disorder called holoprosencephaly The disorder can be caused by a Sonic hedgehog mutation that prevents part of the brain from separating into hemispheres which somehow causes the eyes to form right next to each other, or in this baby's case, fused. The long thing above the eye is the nose but severely deformed and it is called a proboscis. That baby is going to die. Its brain is severely deformed and an organ will probably shut down.
